List of Documents to be Submitted along with the Admission Form

• Registration Form
• Original Birth Certificate with self attested photocopy.
• Aadhaar photocopy of child, father and mother along with original Aadhaar cards for verification.
• Recent passport size colour photograph of child - 4 copies
• Recent passport size colour photograph of father and mother - 1 copy each
• Income Proof Certificate - ITR of finanacial year 2023-2024, or BPL Card or Antyodaya Scheme and Self Declaration Affidavit duly attested by Executive Magistrate
• Transfer Certificate / NOC issued by previous school - Original.
• Photocopy of SC / ST Certificate issued by government authority.
• In case of a single parent, the following documents (photocopies) will be furnished
  

  Divorcee	Divorce decree
  Separated	Legal Separation Document
  Widow / Widower	Death certificate of the spouse

• Student Health Form - Duly filled and signed by a Child Specialist Doctor.
• Photocopy of Vaccination Card - 3 year to 8 year old students.
